Aesthetic goals such as straightening the nasal bridge, refining the tip, or correcting crookedness require consideration of how these structures support one another. Removing more cartilage than necessary or weakening the supporting structures may result over time in drooping of the nasal tip, asymmetry, or breathing problems.

Therefore, the approach of the rhinoplasty surgeon should be much broader than the idea of a “smaller nose.” The proportions of the face in relation to the chin, lips, eyes, and forehead; the thickness of the nasal skin; movement of the nasal tip while smiling; septal deviation; and the condition of the turbinates should all be evaluated together. Instead of applying a similar nose shape to every patient, the aim should be to create a plan that preserves the person’s natural facial expression.

The doctor’s background in ear, nose and throat and head and neck surgery is also valuable in the evaluation of functional problems. Even in a patient seeking aesthetic changes, conditions affecting breathing such as septal deviation, valve narrowing, allergies, or chronic sinusitis may be present. If the surgical plan overlooks these problems, the patient’s expectations may not be fully met even if the appearance improves.

Which evaluations should be performed during the first examination?

A qualified examination begins with the problem the patient sees in the mirror, but it does not end there. The doctor listens to what the patient wants to change about the nose and clearly discusses to what extent this expectation is anatomically possible. Issues such as a hump on the nasal bridge, nasal tip width, crookedness, asymmetry, or deformity related to trauma are evaluated during the external examination.

During the internal examination, the septum, airway, inferior turbinates, and, when necessary, areas related to the sinuses are examined. If there is constant mouth breathing, nighttime obstruction, insufficient airflow during exercise, snoring, or frequent sinusitis attacks, these may change the surgical plan. In some patients, aesthetic rhinoplasty and septoplasty in the same session may be performed; in others, the priority should be correction of the airway.

Photographic analysis is also an important part of planning. Standard images taken from the front, side, below, and oblique angles help evaluate asymmetries and facial proportions more accurately. Digital simulation may be useful for discussing expectations; however, it is not a tool that guarantees the result. Skin healing, tissue memory, and anatomical details encountered during surgery may affect the outcome.

A natural result does not mean the same thing for everyone

Naturalness does not mean that the nose looks completely unchanged. The aim is for the change to be harmonious with the face and for the nose not to become the only feature that draws attention. Small irregularities may be more visible in patients with thin skin, while in patients with thick skin, it may take longer for nasal tip shaping to settle.

At this point, balanced communication between the patient’s wishes and surgical realities is necessary. A very elevated nasal tip or an excessively narrowed nose may not look natural on some faces and may negatively affect the airway. A good plan is prepared not according to trends, but according to the person’s anatomy and long-term tissue behavior.

How should you evaluate the surgeon’s experience?

An academic title alone does not guarantee a result; however, education, scientific work, participation in congresses, and regular surgical experience in a specific field are meaningful indicators when making a decision. Especially a surgeon who focuses on rhinoplasty and revision rhinoplasty is expected to have broader clinical experience with different nose types and possible complications.

The most valuable questions to ask during the examination are those directly related to your own situation: What is the cause of my breathing problem? Is the current cartilage support of my nose sufficient? Which changes are realistic? What should I expect during the recovery process? Are there risks that may require a second procedure? Clear and understandable answers build confidence during the decision-making process.

When reviewing before-and-after photographs, it is important not to focus only on the change seen at first glance. Results in patients with similar facial and skin structures provide a more meaningful idea. It is important that photographs are taken from the same angle, in the same lighting and position, and that the nasal tip, nostrils, and frontal symmetry are also visible. It should be remembered that every surgery is individual and that another patient’s result cannot be copied exactly.

As in Prof. Dr. Gediz Murat Serin’s approach, having the evaluation process conducted by the surgeon personally provides consistency between planning, surgery, and follow-up. This becomes especially important in primary cases accompanied by functional problems and in revision patients with altered anatomy.

Selection is more sensitive in revision rhinoplasty

In a nose that has previously undergone surgery, the structure of the tissues may have changed. Cartilage support may have decreased, firm scar tissue may have formed beneath the skin, or collapse and narrowing affecting the airway may have developed. Therefore, revision rhinoplasty requires more planning and often additional tissue compared with the first surgery.

In some revision cases, it may be necessary to support the nasal framework again with cartilage taken from the ear or rib. Whether this is necessary depends on the extent of the previous surgery, the amount of existing cartilage, and the targeted correction. Not every revision has the same level of difficulty, but even a problem that appears to be a “small touch-up” may be caused by a complex structural issue inside.

Timing is also important. In most cases, enough time should be allowed after the first surgery for swelling to decrease and the tissues to become stable. Except for situations such as infection, severe breathing difficulty, or tissue loss that require urgent intervention, a rushed decision made in the early period may create a greater need for correction in the long term.

Why are hospital infrastructure and postoperative follow-up important?

Although rhinoplasty is generally a planned procedure, safe surgery requires appropriate hospital conditions, anesthesia evaluation, sterilization standards, and multidisciplinary support when necessary. Medications used before surgery, bleeding tendency, allergies, previous operations, and accompanying diseases should be evaluated in detail.

The postoperative process is also part of the result. Swelling, mild drainage, obstruction, and bruising may be seen during the first days. The degree of bruising varies according to the individual and the technique used. Although the shape of the nose improves noticeably during the first weeks, especially the fine healing of the nasal tip continues for months.

Regular follow-up visits allow healing to be monitored correctly. The patient should avoid trauma to the nose, apply the recommended nasal cleaning methods, follow the timing for returning to physical activity, and report unexpected symptoms without delay. Conditions such as worsening pain, high fever, heavy bleeding, or significant discoloration require medical evaluation.

Setting expectations correctly when making a decision

Rhinoplasty is a surgery that can change a person’s facial expression and improve breathing quality; nevertheless, perfect symmetry or achieving another person’s nose exactly are not realistic goals. The human face is naturally asymmetrical, and healing may not progress at exactly the same rate on both sides.

The healthiest decision is made after a detailed evaluation with a specialist who listens carefully to you and clearly explains the limitations and risks as well as the benefits. The right plan for your nose is not the one that promises the most ambitious change, but the one that is most respectful of your face, your breathing, and your long-term quality of life.

When deciding on nose surgery, the choice is not only about how the nose will look in photographs.
