The chronic sinusitis symptoms list helps to understand when these complaints become meaningful together and when an ENT evaluation is needed. Chronic sinusitis is a condition of inflammation and swelling in the tissue lining the sinuses that continues for at least 12 weeks; experiencing only a headache, however, does not by itself suggest this diagnosis.

The sinuses are air-filled cavities inside the facial bones. When the ventilation of these areas connected to the nose or the drainage of secretions is impaired, swelling of the mucosa, discharge, and a sensation of pressure may occur. The severity of complaints varies from person to person. In some patients, the main problem is being unable to breathe, while in others, persistent postnasal drip or loss of smell significantly affects daily life.

İndex

Toggle

Chronic sinusitis symptoms list

The difference between acute sinusitis and chronic sinusitis

Why is the nasal structure important?

In which situations should an ENT specialist be consulted?

How is the diagnosis clarified?

The goal of treatment: Controlling inflammation, preserving breathing

When is sinus surgery considered?

Chronic sinusitis symptoms list

The most valuable symptoms for diagnosis in chronic sinusitis are nasal congestion, thick discharge from the nose or postnasal region, a feeling of pressure or pain in the face, and decreased sense of smell. If at least two of these complaints continue for more than 12 weeks, especially when accompanied by nasal congestion or discharge, a detailed examination is required.

Symptoms often follow a fluctuating course. Congestion that improves for a period may increase again with seasonal allergies, upper respiratory tract infections, or environmental irritants. Findings frequently described by patients include:

Persistent or frequently recurring nasal congestion

Yellow-green, thick nasal discharge or postnasal drip

Decreased sense of smell, perceiving smells differently, or loss of smell

A feeling of fullness and pressure in the cheeks, around the eyes, and forehead area

Cough that may become more noticeable in the mornings, the need to clear the throat, and a sensation of phlegm

Mouth breathing, deterioration in sleep quality, snoring, and daytime fatigue

Headache alone is not a reliable indicator of chronic sinusitis. Migraine, tension-type headache, temporomandibular joint problems, or eye-related problems may cause complaints similar to sinus pain. Therefore, automatically attributing facial pain to sinusitis may lead to a delay in the correct treatment.

The difference between acute sinusitis and chronic sinusitis

Acute sinusitis mostly develops after a viral upper respiratory tract infection and may improve within days to a few weeks. In chronic sinusitis, however, the problem is not simply the prolongation of an infection. Anatomical narrowness affecting the ventilation of the sinuses, nasal polyps, an allergic background, mucosal sensitivity, or inflammation remaining from previous infections may play a role in the continuation of the condition.

This distinction directly affects the choice of treatment. Using antibiotics for every long-lasting discharge is not the correct approach. Antibiotics are beneficial only in certain clinical situations suggesting bacterial infection and with physician evaluation. In chronic sinusitis, the main goal is to identify the underlying causes and support the natural drainage of the sinuses.

Why is the nasal structure important?

Crookedness of the middle partition of the nose, known as septal deviation, enlargement of the turbinates, or a narrow structure in the sinus drainage channels may increase congestion in some patients. However, not every deviation is a cause of chronic sinusitis, and surgery may not be necessary in every patient with chronic sinusitis. The relationship between structural problems and complaints should be evaluated together through examination, endoscopic assessment, and imaging when necessary.

Nasal polyps are also of particular importance. These are soft tissues that develop from the nasal and sinus mucosa, are benign, but may cause congestion and loss of smell. Complaints may be more resistant in patients with polyps; additional conditions such as asthma or sensitivity to certain painkillers should also be questioned.

In which situations should an ENT specialist be consulted?

Nasal congestion, discharge, or loss of smell lasting longer than twelve weeks is a sufficient reason for an ENT examination. Patients who do not experience relief despite frequent antibiotic use, who have similar complaints with every infection, or whose sleep and work performance are significantly affected should not wait.

Some symptoms require faster evaluation. Swelling or redness around the eyes, decreased vision or double vision, severe and unusual headache, neck stiffness, high fever, or confusion require urgent medical evaluation. Detailed examination is also important in cases of one-sided foul-smelling discharge, recurrent nosebleeds, numbness in the face, or progressive congestion on only one side. These findings do not always indicate a serious disease, but they should not be considered simple sinusitis.

How is the diagnosis clarified?

The diagnosis of chronic sinusitis is not made based only on complaints. During the ENT examination, the internal structures of the nose, the nature of the discharge, swelling, presence of polyps, and the septum are evaluated. Nasal endoscopy allows the inside of the nose and the sinus drainage area to be seen in greater detail with the help of a thin camera. This examination is valuable for showing polyps, signs of inflammation, or anatomical narrowness that cannot be noticed from the outside.

Computed tomography is used to evaluate the bone structure of the sinuses, their ventilation, and the extent of the disease. Tomography is not required for every case of nasal congestion. Especially in symptoms that continue despite medical treatment, before surgical planning, or in complex cases, imaging guides the correct decision. Allergy evaluation, smell tests, and additional examinations may also be required in certain specific situations.

The goal of treatment: Controlling inflammation, preserving breathing

The treatment plan is personalized according to the duration of symptoms, endoscopy and tomography findings, and accompanying conditions such as allergies or polyps. Nasal irrigation with saline, when applied with the proper technique, may contribute to removing secretions and moisturizing the mucosa. Intranasal corticosteroid sprays recommended by the physician are an important component of treatment, especially in chronic inflammation and a tendency toward polyps.

Oral medications, allergy treatment, or different short-term treatments may be necessary in some patients. On the other hand, long-term and uncontrolled use of nasal decongestant drops may cause a more difficult condition called rebound congestion. The duration, dosage, and combination of medications should therefore be determined individually.

When is sinus surgery considered?

If complaints continue despite appropriate medical treatment, if polyps obstruct airflow, or if there are significant structural problems impairing sinus drainage on tomography, endoscopic sinus surgery may be considered. The aim of this approach is to functionally regulate the natural openings of the sinuses, clean diseased tissue to the necessary extent, and facilitate the delivery of medical treatment to the sinus mucosa.

The decision for surgery is not made only according to imaging results. The patient’s breathing capacity, loss of smell, frequency of infections, quality of life, nasal anatomy, and response to previous treatments are evaluated together. In patients planning nasal aesthetic surgery or septum surgery, evaluating respiratory function and sinus health within the same assessment is also valuable for long-term results.

Chronic sinusitis is often a manageable disease; the correct approach is to make the causes visible rather than temporarily suppressing the complaint. Instead of considering long-lasting congestion an unavoidable part of daily life, learning through a comprehensive ENT evaluation whether it is possible to breathe more comfortably and sleep better is a good starting point.

If nasal congestion has been continuing for weeks, the sense of smell has decreased, and postnasal drip irritates the throat throughout the day, something more than a simple cold may be considered.
