class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>One of the patient groups I frequently encounter in my clinical practice are those who have been treated for &#8220;allergies&#8221; for years, yet their tests come back negative, and despite this, their complaints of nasal congestion, discharge, and postnasal drip continue. This situation often suggests a group of diseases called <strong>non-allergic rhinitis<\/strong>.<\/p>\n<p>Non-allergic rhinitis encompasses chronic nasal diseases that occur without allergy, infection, or a specific microbial cause. While the most common type is <strong>vasomotor rhinitis<\/strong> (idiopathic rhinitis), there are many subtypes that develop through different mechanisms.<\/p>\n<p>Treatments administered without proper diagnosis are often inadequate. Therefore, detailed evaluation and personalized approach are of great importance.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_Non-Allergic_Rhinitis\"><\/span><strong>What is Non-Allergic Rhinitis?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In non-allergic rhinitis, vascular dilation and increased secretion are seen in the nasal mucosa. Patients typically have the following allergy-like complaints:<\/p>\n<ul>\n<li>Continuous or periodic nasal congestion<\/li>\n<li>Clear and watery nasal discharge<\/li>\n<li>Postnasal drip<\/li>\n<li>Frequent throat clearing<\/li>\n<li>Cough<\/li>\n<li>Feeling of fullness in the nose<\/li>\n<\/ul>\n<p>These symptoms often continue throughout the year and are not seasonal.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Vasomotor_Rhinitis_The_Most_Common_Cause\"><\/span>Vasomotor Rhinitis: The Most Common Cause<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Vasomotor rhinitis develops as a result of an imbalance in the autonomic nervous system that controls the nasal mucosa.<\/p>\n<p>Normally:<\/p>\n<ul>\n<li>The parasympathetic system increases secretion<\/li>\n<li>The sympathetic system constricts blood vessels<\/li>\n<\/ul>\n<p>When the balance is disrupted:<\/p>\n<ul>\n<li>Nasal blood vessels dilate<\/li>\n<li>Edema forms in the mucosa<\/li>\n<li>Watery discharge and congestion develop<\/li>\n<\/ul>\n<p>Patients generally fall into two groups:<\/p>\n<ul>\n<li><strong>Those with predominant discharge<\/strong><\/li>\n<li><strong>Those with predominant congestion<\/strong><\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Factors_That_Worsen_Symptoms\"><\/span>Factors That Worsen Symptoms<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In non-allergic rhinitis, environmental irritants play a role more than allergens:<\/p>\n<ul>\n<li>Perfume and chemical odors<\/li>\n<li>Cigarette smoke<\/li>\n<li>Cold air and sudden temperature changes<\/li>\n<li>Humidity and pressure changes<\/li>\n<li>Alcohol<\/li>\n<li>Spicy foods<\/li>\n<li>Stress<\/li>\n<\/ul>\n<p>Patients typically express this as &#8220;I suddenly get worse in certain environments.&#8221;<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Other_Subtypes_of_Non-Allergic_Rhinitis\"><\/span>Other Subtypes of Non-Allergic Rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The following causes should also be considered in clinical evaluation:<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Drug-induced_rhinitis\"><\/span>Drug-induced rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Prolonged use of nasal decongestant sprays (rhinitis medicamentosa)<\/p>\n<h3><span class=\"ez-toc-section\" id=\"NARES\"><\/span>NARES<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Eosinophil increase despite negative allergy tests<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Hormonal_rhinitis\"><\/span>Hormonal rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Pregnancy or thyroid diseases<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Senile_rhinitis\"><\/span>Senile rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>In advanced age, discharge increasing especially during meals<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Gustatory_rhinitis\"><\/span>Gustatory rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Watery discharge triggered by eating<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Occupational_rhinitis\"><\/span>Occupational rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Chemical and irritant exposure<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Atrophic_rhinitis\"><\/span>Atrophic rhinitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><strong>Important: Differential Diagnosis<\/strong><\/p>\n<p>Before diagnosing non-allergic rhinitis, the following conditions must be excluded:<\/p>\n<ul>\n<li>Allergic rhinitis<\/li>\n<li>Chronic sinusitis<\/li>\n<li>Nasal polyp or mass<\/li>\n<li>Septal deviation<\/li>\n<li>Turbinate hypertrophy<\/li>\n<\/ul>\n<p>If there is unilateral and continuous clear discharge, evaluation for <strong>cerebrospinal fluid leak<\/strong> is necessary.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Diagnostic_Process\"><\/span>Diagnostic Process<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In my own practice, diagnosis is evaluated in three steps:<\/p>\n<ol>\n<li>Detailed patient history<\/li>\n<li>Nasal endoscopic examination<\/li>\n<li>If necessary:\n<ul>\n<li>Allergy tests<\/li>\n<li>Nasal cytology<\/li>\n<li>Sinus tomography<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p>On endoscopy, pale and edematous mucosa with clear secretion is typically observed.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Treatment_Approach\"><\/span>Treatment Approach<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<ol>\n<li>\n<h3><span class=\"ez-toc-section\" id=\"Trigger_Control\"><\/span><strong>Trigger Control<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<\/li>\n<\/ol>\n<ul>\n<li>Avoiding chemical odors<\/li>\n<li>Staying away from cigarette smoke<\/li>\n<li>Protection from sudden temperature changes<\/li>\n<li>Limiting alcohol and spices<\/li>\n<\/ul>\n<ol start=\"2\">\n<li>\n<h3><span class=\"ez-toc-section\" id=\"Medical_Treatment\"><\/span><strong>Medical Treatment<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<\/li>\n<\/ol>\n<p><strong>Corticosteroid nasal sprays<\/strong><br \/>\nThis is the basic and most effective treatment. It is used for a long time.<\/p>\n<p><strong>Ipratropium bromide<\/strong><br \/>\nEffective in patients with predominant discharge.<\/p>\n<p><strong>Topical antihistamines<\/strong><br \/>\nBeneficial in combined complaints.<\/p>\n<p>The best results are usually obtained with combination therapy.<\/p>\n<ol start=\"3\">\n<li>\n<h3><span class=\"ez-toc-section\" id=\"Important_Warning\"><\/span><strong>Important Warning<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<\/li>\n<\/ol>\n<p>When nasal decongestant sprays are used for more than 5 days, <strong>medication dependency and rebound congestion<\/strong> may develop.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Surgical_Options_for_Resistant_Patients\"><\/span>Surgical Options for Resistant Patients<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In patients who do not improve despite medication:<\/p>\n<ul>\n<li>Mucosa-preserving <strong>inferior turbinate reduction<\/strong><\/li>\n<li>Advanced functional surgeries in selected cases<\/li>\n<\/ul>\n<p>These procedures increase the effectiveness of medical treatment by expanding the airway.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"An_Important_Point_from_Clinical_Experience\"><\/span>An Important Point from Clinical Experience<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A significant portion of non-allergic rhinitis patients have accompanying structural problems (septal deviation, turbinate hypertrophy, valve insufficiency).
