Through the thyroid hormones it produces, it maintains many systems from the heart&#8217;s working rate to body temperature, from digestion tempo to muscle strength.<br \/>\n<strong>Goiter<\/strong> is the enlargement of this gland beyond normal size for various reasons.<br \/>\nThis enlargement may be limited to a single area or may spread throughout the entire gland. As it enlarges, it begins to press on the vital structures around it:<\/p>\n<ul>\n<li>Pressure on the trachea \u2192 shortness of breath, especially increased respiratory difficulty when lying on the back<\/li>\n<li>Pressure on the esophagus \u2192 difficulty swallowing, feeling of something stuck in the throat<\/li>\n<li>Pressure on neck soft tissues \u2192 tension, fullness and discomfort<\/li>\n<\/ul>\n<p>Patients often express this as &#8220;there&#8217;s something in my neck&#8221;, &#8220;I have difficulty swallowing&#8221; or &#8220;my breathing becomes difficult when I lie down&#8221;.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Why_Does_Goiter_Occur\"><\/span>Why Does Goiter Occur?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Goiter is not a single disease, but a result produced by different processes. The most common causes are:<\/p>\n<ul>\n<li>Growth of thyroid nodules over time<\/li>\n<li>Development of multinodular goiter<\/li>\n<li>Immune system-related diseases such as Graves&#8217; disease<\/li>\n<li>Prolonged stimulation or inflammation of thyroid tissue<\/li>\n<li>Family predisposition<\/li>\n<li>Previous radiation to the neck area<\/li>\n<\/ul>\n<p>Although iodine deficiency is still an important cause worldwide, it is less common in urban life today.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"How_is_Goiter_Evaluated\"><\/span>How is Goiter Evaluated?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Evaluation is performed under two main headings:<\/p>\n<ol>\n<li>\n<h3><span class=\"ez-toc-section\" id=\"Evaluation_of_hormones\"><\/span>Evaluation of hormones<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<\/li>\n<\/ol>\n<ul>\n<li>TSH<\/li>\n<li>Free T3<\/li>\n<li>Free T4<\/li>\n<\/ul>\n<ol start=\"2\">\n<li>\n<h3><span class=\"ez-toc-section\" id=\"Evaluation_of_tissue_structure\"><\/span>Evaluation of tissue structure<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<\/li>\n<\/ol>\n<ul>\n<li>Thyroid ultrasonography<\/li>\n<li>Fine needle aspiration biopsy if necessary<\/li>\n<li>Computed tomography for large or deeply extending goiters<\/li>\n<\/ul>\n<p>The aim is to clarify whether the goiter is benign or risky and whether it disrupts hormone balance.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"How_is_Goiter_Treated\"><\/span>How is Goiter Treated?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Not every goiter necessarily requires treatment. The decision on whether treatment is needed is made with these three questions:<\/p>\n<ul>\n<li>Is it causing complaints?<\/li>\n<li>Is it continuing to grow?<\/li>\n<li>Does it carry risky features?<\/li>\n<\/ul>\n<p>Accordingly, the following options are evaluated:<\/p>\n<ul>\n<li>Follow-up<\/li>\n<li>Medical treatment<\/li>\n<li>Radioactive iodine (&#8220;atomic therapy&#8221;)<\/li>\n<li>Surgery (thyroid surgery)<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"When_is_Surgery_Necessary\"><\/span>When is Surgery Necessary?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Surgery becomes relevant in the following situations:<\/p>\n<ul>\n<li>If respiratory or swallowing function is affected<\/li>\n<li>If the goiter is growing rapidly<\/li>\n<li>If there is suspicion of cancer<\/li>\n<li>If substernal goiter is present<\/li>\n<li>If the patient is significantly bothered by cosmetic or pressure complaints<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Conclusion\"><\/span>Conclusion<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Goiter is usually benign; however, not every enlargement is innocent. A palpable mass in the neck, a gradually increasing feeling of fullness, or breathing-swallowing problems should definitely be evaluated.
