However, the biggest mistake is evaluating the condition with a CT scan right at the onset or hastily starting aggressive treatments. In this article, we will explain the correct timing for diagnosis and treatment in acute sinusitis, common mistakes, and when advanced testing is necessary.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"When_Is_a_CT_Scan_Necessary_in_Acute_Sinusitis\"><\/span>When Is a CT Scan Necessary in Acute Sinusitis?<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>One of the most frequently asked questions by patients is whether <strong>computed tomography (CT)<\/strong> is needed for the diagnosis of sinusitis. However, it should be noted that <strong>performing a CT scan at the onset of acute sinusitis is not an appropriate approach<\/strong>. Because:<\/p>\n<ul>\n<li>In a newly started infection, edema may be observed in the sinuses, which may not always reflect an actual inflammation.<\/li>\n<li>A CT scan taken within 7\u201310 days after treatment begins is too early to reflect the course of the disease and may be misleading.<\/li>\n<li>Moreover, CT scans can lead to <strong>unnecessary radiation exposure<\/strong>, especially in children and young adults.<\/li>\n<\/ul>\n<p>Therefore, CT should only be considered in the following cases:<\/p>\n<ul>\n<li>In cases of <strong>chronic sinusitis lasting longer than 12 weeks<\/strong> or frequently recurring sinusitis,<\/li>\n<li>If <strong>eye complications<\/strong> such as redness or swelling around the eyes develop,<\/li>\n<li>If there is suspicion of <strong>brain complications<\/strong> such as severe headache or changes in consciousness.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Most_Common_Microbes_in_Acute_Sinusitis\"><\/span>Most Common Microbes in Acute Sinusitis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The three most commonly encountered bacteria in acute sinusitis are:<\/p>\n<ul>\n<li><em>Streptococcus pneumoniae<\/em><\/li>\n<li><em>Haemophilus influenzae<\/em><\/li>\n<li><em>Moraxella catarrhalis<\/em><\/li>\n<\/ul>\n<p>Unfortunately, it is often not possible to directly obtain cultures from the sinuses. However, if the patient shows a <strong>poor clinical response<\/strong>, the antibiotic may need to be changed.<\/p>\n<p>In some patients, for instance, <em>Strep. pneumoniae<\/em> may be dominant, while in others, different bacteria may be more prominent. This can affect the <strong>choice of antibiotics<\/strong> during the treatment process.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Is_Antibiotic_Treatment_Necessary\"><\/span>Is Antibiotic Treatment Necessary?<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Antibiotics are not necessary for every sinusitis case. Especially during the summer months, in patients exposed to <strong>sea water<\/strong> or saltwater entering the nose, the issue may be only nasal irritation or temporary mucosal edema. In particularly resistant cases, the following supportive approaches are of great importance:<\/p>\n<ul>\n<li>Nasal rinses (isotonic or hypertonic saline)<\/li>\n<li><strong>Nasal steroid sprays<\/strong> (for anti-inflammatory effect)<\/li>\n<li>Antihistamine treatments (especially if there is a history of allergy)<\/li>\n<li><strong>Decongestant nasal sprays<\/strong> (only for short-term use)<\/li>\n<\/ul>\n<p>In some cases, if the disease does not resolve despite treatment, changing the antibiotic may be necessary. In such cases, the physician should re-evaluate the patient and, if necessary, switch to <strong>alternative antibiotic agents<\/strong>.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Anatomical_and_Immune_Factors\"><\/span>Anatomical and Immune Factors<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Each patient&#8217;s condition is different.
